Preparing for your Visit
To make your experience as smooth and efficient as possible, we have streamlined our registration process. Please review the following steps to ensure you are ready for your upcoming appointment.
Pre-Visit Digital Registration
We utilize a secure digital system to allow you to check in from the comfort of your home.
- Watch for the Link: Approximately 3–5 days before your appointment, you will receive a secure registration link via text or email.
- Complete at Home: We strongly encourage you to complete this before you arrive. You will be able to:
- Update your contact information and insurance details.
- Electronically sign required consent forms and financial policies. o Upload photos of your ID and insurance cards.
- Complete clinical screenings and pay your co-payment.
What to Bring with You
Even if you have completed your registration digitally, our staff must perform a final verification to ensure billing accuracy and patient safety. Please bring the following to every visit:
- Photo Identification: A valid driver’s license or government-issued ID.
- Current Insurance Card: We must verify your active coverage and policy numbers at every visit to prevent claim rejections.
Arriving for Your Appointment
Please arrive at least 15 minutes prior to your appointment time, and check in at the front desk.
- If you didn’t pre-register: Don't worry! We will provide you with a tablet or a mobile code to complete your check-in while you wait.
- Data Verification: Our team will verify your legal name, date of birth, and current mailing address to ensure your medical record is up to date.
Financial Information & Assistance
- Co-payments: Co-payments are due at the time of service. You may pay these during your digital pre-registration or at the front desk upon arrival.
- Insurance Coverage: If your insurance cannot be verified at the time of your visit, you will be registered as "Self-Pay" until coverage is confirmed.
- Financial Assistance: We are dedicated to caring for all members of our community. If you are concerned about the cost of your care, please ask to speak with our Patient Advocate. We offer a Sliding Fee Scale for eligible patients based on income and family size.
