Free Sports Physicals Offered August 4th and August 6th!
Goodland Family Health Center will be offering free sports physicals to students who plan to participate in organized sports in the 2020-2021 school year. Due to COVID-19, we are having two separate days of free sports physical clinics to help with social distancing. Please see below.
August 4, 2020 at 4 pm MST– students with last names A-M & kids in same household
August 6, 2020 at 4 pm MST –students with last names N-Z & kids in same household
Please remember that while in the clinic we are asking that everyone please practice social distancing (6 feet apart), wear a mask, to wash your hands frequently (hand sanitizer will be available), and please DO NOT come if you are experiencing cough, shortness of breath, muscle aches, nausea, headache, fever, chills, runny nose or nasal congestion, diarrhea, or changes in taste or smell.
Also, please bring your completed form with you (including your parents’ or guardian’s signature).
